A neck lift is an operation that restores definition to the neck and jawline by tightening the platysma muscle, removing excess fat and redraping or removing loose skin. Doctors often call it a platysmaplasty or lower rhytidectomy. It treats vertical neck bands, a full or heavy neck, a double chin that does not respond to diet or exercise, and the loss of the sharp angle between the jaw and the neck that most people associate with a youthful profile.

The neck is often the area that bothers people most, and it is also one of the hardest to disguise. Clothing, make-up and skin treatments make little difference to it. Photographs in profile, video calls and lowered head positions all show it. Because the neck ages in layers, the operation needs to address each of those layers in turn.

Why the neck ages

Three separate changes combine as the neck ages:

  1. The platysma muscle loosens. This thin sheet of muscle runs from the collarbone up over the jaw. With age, its inner edges separate and relax, creating the two vertical cords often called neck bands.
  2. Fat accumulates. Fat can gather above the muscle, where liposuction can reach it, and beneath it, where it cannot. Deep fat and prominent salivary glands contribute to a heavy neck that does not respond to weight loss.
  3. The skin loses elasticity. Loose skin gathers in folds and no longer follows the contour of the jaw.

A neck lift is planned around which of these is most responsible for your neck. A younger patient with fat but good skin may need only double chin liposuction. An older patient with loose skin and bands needs a full neck lift, often combined with a facelift.

Who is a good candidate?

You may be a good candidate if you have:

  • visible vertical bands when you speak or tense the neck
  • loose skin under the chin and along the jaw
  • fat that remains under the chin despite a healthy weight
  • a blurred or obtuse angle between the jaw and the neck in profile

You should be in good general health, a non-smoker or willing to stop nicotine for at least four weeks before and after the operation, and your weight should be stable. Bone structure matters too. A small, set-back chin limits how sharp a neck can become, and in some patients a chin implant or other chin surgery is discussed as part of the plan.

How a neck lift is performed

The operation is usually carried out under general anaesthesia or deep sedation. It generally follows these steps:

  1. Access under the chin. A short incision is made in the natural crease below the chin. Through it, the fat above the muscle is sculpted and, where appropriate, excess fat beneath the muscle is reduced.
  2. Muscle tightening. The separated inner edges of the platysma are brought together in the midline with stitches, like tightening a corset. This removes the bands and creates a firm sling under the jaw.
  3. Lateral lift. Through incisions around the ears, the sides of the platysma are lifted upwards and backwards, which sharpens the jawline and defines the angle of the neck.
  4. Skin redraping. The skin is redraped over the new contour and any excess is removed behind the ear, where the scar is hidden in the hairline.

When the lower face is also treated, the lateral part of the neck lift is performed as part of a facelift or deep plane facelift through the same incisions.

Bleeding beneath the skin is one of the main concerns after neck surgery. A hemostatic net, a temporary pattern of stitches that holds the skin firmly against the tissue beneath, may be used to lower this risk.

Neck lift options compared

Approach What it treats Incisions Typical candidate
Neck liposuction Fat above the muscle 2–3 tiny incisions Younger, good skin
Anterior platysmaplasty Bands, fat under the chin Under the chin only Bands with little loose skin
Full neck lift Bands, fat and loose skin Under chin and around ears Moderate to advanced ageing
Neck lift with facelift Neck, jawline and jowls Around ears and under chin Ageing of lower face and neck together

The role of the chin and jaw

A sharp neck depends on the skeleton as much as on the soft tissue. The lower jaw and chin act as the frame the neck hangs from. When the chin is small or set back, even a well-tightened neck can look soft in profile, because there is not enough projection to create a clean angle. During the examination your profile is assessed with this in mind. In some patients, improving chin projection with an implant or fat transfer is suggested alongside the neck lift. In others, the jaw is strong enough and no change is needed. It is a detail that often makes the difference between a good and an excellent neck result.

Preparing for surgery

Before the operation you will have blood tests and an anaesthetic review. Stop all nicotine products at least four weeks beforehand. Medicines and supplements that affect bleeding, such as aspirin, anti-inflammatory painkillers, fish oil, vitamin E and ginkgo, are usually paused for two weeks, after checking with the prescribing doctor. Men are generally asked to keep a short beard or be clean-shaven, as a long beard makes dressings and wound care harder. Buttoned shirts are easier to wear than clothes that go over the head in the first days.

Recovery after a neck lift

You will wake with a light compression dressing around the neck and jaw. Most patients go home the same day or after one night. A compression garment is worn continuously for the first week and then at night for another week or so.

  • Days 1–3: tightness, swelling and some difficulty turning the head; rest with the head raised
  • Days 5–7: swelling settling; stitches under the chin removed
  • Days 10–14: most bruising gone; return to work and light activities
  • Weeks 3–4: light exercise; avoid bending the neck forcefully
  • Months 3–6: firmness and numbness under the chin fade; final contour visible

A feeling of tightness when swallowing or bending the head forward is common for a few weeks. It is caused by the tightened muscle and fades gradually.

Risks

As with any surgery, there are risks. They include haematoma, infection, delayed healing, visible scars, temporary numbness, irregularities in the contour, and, uncommonly, temporary weakness of the nerve branch that moves the lower lip, which can make the smile look uneven for a few weeks. Over-removal of fat can leave the neck looking hollow, which is why the fat is sculpted conservatively.

Blood pressure control, stopping nicotine and following the compression and positioning instructions all lower the risk of complications.

Having a neck lift in Istanbul

If you are travelling for surgery, plan to spend about a week in Istanbul. This covers the examination, surgery, the first dressing change and stitch removal. Combined with a facelift, allow eight to ten days. When flying home, wear your compression garment, move around the cabin and drink plenty of water.

Dr. Mehmet Özdemir is a plastic, reconstructive and aesthetic surgeon with the EBOPRAS qualification of the European Board of Plastic, Reconstructive and Aesthetic Surgery. During consultation he examines the neck at rest and in motion, feels the fat and muscle layers, and explains which combination of treatments will give you the jaw-neck angle you are hoping for.

Neck Lift: frequently asked questions

Can I have a neck lift without a facelift?

Yes, if the jowls and lower face are still in reasonable shape and the ageing is concentrated below the jaw. Many people, though, have some jowling as well, and treating the neck alone can leave a visible step between a tight neck and a soft jawline. Both areas are examined before a decision is made.

Will a neck lift get rid of "turkey neck"?

A "turkey neck" usually combines loose skin with vertical muscle bands. A neck lift that tightens the platysma muscle and removes the excess skin is the most reliable way to treat it. Injections can soften mild bands but cannot remove loose skin.

Is liposuction enough for a double chin?

For younger patients with good skin elasticity and fat that sits above the muscle, liposuction alone can work well. When the skin is loose, the muscle has separated or fat lies beneath the muscle, liposuction on its own is not enough and a neck lift gives a better and longer-lasting result.

Where are the scars after a neck lift?

Usually a short incision in the natural crease under the chin, and incisions that run around and behind the ears when skin needs to be removed. Once healed, they are hidden by the shadow of the jaw and by the hair behind the ears.

How long do neck lift results last?

The muscle tightening and skin removal give results that typically last many years. The neck will continue to age slowly, but it starts from a more defined baseline. Keeping a stable weight helps preserve the result.
