Double chin liposuction, or submental liposuction, is a procedure that removes excess fat from under the chin and along the upper neck and jawline through a few tiny incisions. It sharpens the angle between the jaw and the neck and gives the lower face a more defined outline. It is one of the simplest and most effective facial contouring procedures when the problem is fat rather than loose skin or muscle.

Many people carry a double chin even at a healthy weight. It can be hereditary, and the fat under the chin often resists diet and exercise. Because it shows in profile and in photographs taken from slightly below, it tends to bother people more than its size would suggest.

Is it fat, skin or muscle?

The fullness under the chin can come from different sources, and the right treatment depends on which one is responsible.

Cause What it looks like Best treatment
Fat above the neck muscle Soft fullness that can be pinched Double chin liposuction
Loose skin Folds that hang, especially when looking down Neck lift
Separated neck muscle Vertical bands when speaking Neck lift with muscle tightening
Deep fat under the muscle Firm fullness that cannot be pinched Neck lift with deep fat reduction
Weak chin or low hyoid bone Blunted angle even without much fat Chin augmentation, sometimes with liposuction

A pinch test gives a first idea. If you can gather a thick fold of soft tissue under the chin and your skin snaps back well when released, liposuction is likely to help. If the skin stays loose, or you see bands when you tense your neck, a neck lift is probably the better choice.

Who is a good candidate?

You may be a good candidate if you:

  • have fat under the chin that persists at a healthy weight
  • have good skin elasticity, usually more common in younger people
  • have no significant muscle bands or loose skin
  • are in good general health and at a stable weight

Age is less important than skin quality. Some people in their fifties have excellent elasticity, while some in their thirties already have loose skin.

How the procedure is performed

The procedure usually takes thirty minutes to an hour. It is commonly done under local anaesthesia, with or without sedation, and can be combined with other procedures under general anaesthesia.

  1. Tumescent solution. A fluid containing local anaesthetic and a medicine that narrows blood vessels is infiltrated into the area. This numbs the tissue and reduces bleeding and bruising.
  2. Small incisions. Tiny incisions are made under the chin and sometimes behind each earlobe.
  3. Fat removal. A thin cannula is moved in a controlled fan pattern to remove fat evenly from under the chin and along the jawline.
  4. Sculpting. A thin layer of fat is left under the skin to keep the contour smooth and to avoid a hollow or skeletal appearance.
  5. Closure. The incisions are closed with a stitch or left to heal, and a chin strap is applied.

In some cases, especially in combination with a facelift, an energy-based device may be used to help the skin tighten, but the basic technique remains careful, even fat removal.

Combining with other procedures

Double chin liposuction is often part of a lower face plan. Common combinations include:

  • chin augmentation for a weak chin, which improves the jaw-neck angle
  • buccal fat removal to slim the lower cheeks
  • a facelift or neck lift when there is also loose skin or jowling
  • botulinum toxin for mild neck bands

Surgical or non-surgical?

Non-surgical options for a double chin exist. Fat-dissolving injections based on deoxycholic acid break down fat cells over a series of treatments, usually two to four sessions a month or more apart. Cooling devices that freeze fat are another option. Both can work for small amounts of fat, but they involve several sessions, noticeable swelling after injections and less predictable results. Liposuction removes the fat in one procedure and allows the surgeon to sculpt the jawline precisely. For a moderate or larger double chin, liposuction is usually more efficient. For a very small pocket of fat in someone who wants to avoid any incision, injections are a reasonable alternative.

Chin liposuction for men

Men often want a stronger, more angular jawline rather than just a smaller chin. Liposuction in men is therefore planned to define the lower border of the jaw and the angle behind it, sometimes with fat removal along the jawline towards the ear. The thicker skin and beard in men tend to hide small irregularities and help the skin contract well. When the chin itself is weak, chin augmentation often makes a bigger difference to the profile than fat removal alone.

Preparing for the procedure

Stop aspirin, anti-inflammatory painkillers and blood-thinning supplements about two weeks beforehand, after speaking to your doctor. Stop smoking. Men should shave the neck on the morning of surgery or keep a short beard. Plan to wear loose, button-front clothes for the first days.

Recovery

  1. Days 1–3: swelling, tightness and some bruising; chin strap worn continuously; sleep with your head raised
  2. Days 3–5: most people return to work; the strap can be hidden with a scarf or high collar
  3. Week 2: most bruising gone; strap worn mainly at night
  4. Weeks 3–4: light exercise resumes; swelling continues to settle
  5. Month 3: the final contour is visible

The skin under the chin may feel firm, lumpy or numb for several weeks. This is normal and settles as the tissues heal. Gentle massage may be recommended.

Risks

Chin liposuction is generally safe, but, as with any procedure, there are risks. They include swelling, bruising, infection, irregular contour, loose skin if elasticity is poor, temporary numbness, and, uncommonly, temporary weakness of the small nerve branch that moves the lower lip, which usually recovers. Removing too much fat can leave the neck looking hollow or show the underlying structures, which is why a conservative, even approach is used.

How long the result lasts

The fat cells removed do not return. As long as your weight stays reasonably stable, the improvement is long-lasting. The skin and neck will continue to age, and some softening may appear over the years, but the chin rarely returns to its previous fullness.

Double chin liposuction in Istanbul

As a short procedure with a quick recovery, double chin liposuction fits into a stay of four to five days, including a check before you fly. Wear your chin strap during the journey if advised.

Dr. Mehmet Özdemir is a plastic, reconstructive and aesthetic surgeon holding the EBOPRAS qualification, trained in Germany, Poland and Brazil, with body contouring and facial surgery among his main areas of focus. He practises in Bakırköy, Istanbul, and consults in Turkish, Kurdish, English and German. At consultation he examines the skin, fat and muscle layers of your neck to confirm whether liposuction alone will give the definition you are hoping for.

Double Chin Liposuction: frequently asked questions

Is double chin liposuction permanent?

The fat cells removed do not grow back. If you gain a significant amount of weight later, the remaining fat cells in the area can enlarge, but the double chin rarely returns to its original size. Keeping a stable weight helps preserve the result.

Will my skin sag after chin liposuction?

In people with good skin elasticity, the skin contracts smoothly over the new contour. If the skin is already loose, liposuction alone can leave it looking slack. In that case a neck lift, which tightens the muscle and removes excess skin, is more appropriate.

How long do I need to wear the chin strap?

Usually continuously for about one week, then mainly at night for a further one to two weeks. The strap helps the skin settle onto the new contour and controls swelling.

Are there scars?

Incisions are only a few millimetres long, typically one under the chin and sometimes one behind each earlobe. They heal into small marks that are hard to see once mature.

Can I have chin liposuction if I am overweight?

It is possible, but the result is usually better at or near a stable, healthy weight. If you are planning to lose weight, doing so first lets the surgeon judge how much fat remains under the chin and how the skin responds. In significant weight loss, loose skin may make a neck lift more suitable.

Is chin liposuction painful?

Most people describe soreness and tightness rather than pain, similar to a bruise. Discomfort usually eases within a few days and is managed with standard painkillers.
