Fat transfer breast augmentation is a way of enlarging the breasts using your own fat instead of implants. Fat is removed by gentle liposuction from an area such as the abdomen, flanks or thighs, purified, and then injected in small amounts into the breasts. It gives a soft, natural result, avoids a foreign implant and slims the donor area at the same time. The main limitation is that the increase in size is modest, usually around one cup size per session.

Women choose fat transfer for different reasons. Some do not want an implant in their body. Some want only a subtle change. Some want to correct asymmetry or a small area of deficiency. Others are having their implants removed and want to restore some of the volume naturally. For these goals it can be an excellent option. For women who want a large change in size, implants are usually the more predictable choice.

What fat transfer can achieve

Goal Is fat transfer suitable?
A modest increase in size Yes, around one cup per session
Improving upper breast fullness Often, in combination with good skin
Correcting mild asymmetry Yes, fat can be added to the smaller side
Softening implant edges or rippling Yes, often combined with implants
Restoring volume after implant removal Partly, depending on how much was lost
Lifting sagging breasts No, a breast lift is needed
A large increase in size Rarely in one session, implants are more reliable

Who is a good candidate?

You may be a good candidate if you:

  • want a natural, modest increase in breast size
  • have enough spare fat in areas such as the abdomen, flanks or thighs
  • have breasts that are not significantly sagging
  • are at a stable weight
  • are a non-smoker and in good health
  • have had normal breast screening appropriate to your age

A mammogram or ultrasound before surgery may be requested, especially if you are over forty or have a family history of breast disease.

How the procedure works

The operation is carried out under general anaesthesia and usually takes two to three hours.

  1. Liposuction. Fat is removed from the chosen donor areas with thin cannulas and low suction pressure, which protects the fat cells. The donor area is contoured at the same time.
  2. Processing. The fat is purified to separate healthy fat cells from fluid, blood and oil.
  3. Injection. The fat is placed in the breast in very small amounts through fine cannulas, in many layers and directions. Each tiny deposit needs to be close to a blood supply to survive, which is why the fat is spread out carefully rather than injected in large pockets.

The amount of fat that can be placed is limited by the space available. Overfilling the breast in one session reduces survival and increases the risk of cysts, so it is better to plan a second session if more volume is needed.

Preparing the breast

In some women with tight skin, the breast tissue has limited space to receive fat. Techniques such as external expansion devices worn before surgery have been used to create more space, although they require commitment over several weeks. In most cases, careful technique and realistic goals are enough. This is discussed at consultation.

Fat survival

Not all transferred fat survives. In the first weeks, the body reabsorbs part of it, and the rest establishes a blood supply and becomes permanent. The final result is usually assessed at around six months. Many patients see a noticeable but modest increase. Some choose a second session for more volume. Smoking and weight loss after surgery both reduce the final result.

Fat transfer and breast imaging

Fat grafting can cause small oil cysts and calcifications within the breast. These are usually harmless, but they can appear on mammograms and ultrasound. Radiologists familiar with fat grafting can generally tell them apart from changes that need further investigation, but occasionally a biopsy is recommended to be certain. Always tell your radiologist that you have had fat transfer. Current evidence has not shown an increased risk of breast cancer from fat grafting to the breast.

Preparing for surgery

Stop smoking at least four weeks before and after surgery. Stop blood-thinning medicines and supplements two weeks beforehand, after speaking to your doctor. Keep your weight stable, as a large change afterwards will affect both the breasts and the donor area. A compression garment for the donor area and a soft support bra will be provided or recommended.

Recovery

  1. Days 1–3: the donor area is sore and bruised, like after liposuction; the breasts are swollen
  2. Days 3–7: discomfort improves; compression garment worn on the donor area
  3. Weeks 1–2: most people return to desk work
  4. Weeks 2–6: avoid pressure on the breasts, including sleeping on your front and tight underwired bras
  5. Months 3–6: the fat that will survive has established itself

The breasts may look fuller in the first weeks due to swelling and the fat that will later be reabsorbed. This is expected.

Risks

Possible risks include bruising and swelling, lumps or firmness from fat necrosis, oil cysts, calcifications, reabsorption of more fat than expected, asymmetry, infection, and irregularities in the donor area. Serious complications are uncommon.

Fat transfer after implant removal

A growing number of women choose to have their implants removed, whether because of complications, a change in preference or concerns about long-term implant safety. After removal, the breasts can look deflated, especially if the implants were large. Fat transfer can restore some of this volume, either at the same time as implant removal or a few months later once the tissues have settled. Doing it later is often more reliable, as the breast pocket has closed and the fat is placed into healthy tissue rather than around a fresh cavity. A lift may also be needed when the skin has been stretched. The final volume is usually smaller than with the implants, but many women prefer the natural softness.

Combining fat with implants

For women who want more volume than fat alone can provide but prefer a softer, more natural look, implants and fat can be combined. The implant provides the main increase, and a thin layer of fat around it softens the edges and improves the transition from the chest to the breast, particularly in slim women. This is sometimes called hybrid or composite augmentation. More on implants is on the breast augmentation page.

Fat transfer breast augmentation in Istanbul

International patients usually stay six to seven days. This covers the assessment, surgery and a check-up. Wear your compression garment during the flight and walk around the cabin regularly.

Fat Transfer Breast Augmentation: frequently asked questions

How much bigger can my breasts become with fat transfer?

Usually around one cup size per session, depending on how much fat you have, how much the skin can accommodate and how much fat survives. Larger increases generally need a second session several months later, or implants.

How much of the fat survives?

A proportion of the transferred fat is reabsorbed in the first months. What survives after about six months is considered permanent. Survival varies between individuals and depends on technique, on how the fat is placed and on the tissue receiving it. Smoking markedly reduces it.

Does fat transfer to the breast affect mammograms?

Fat grafting can cause small oil cysts or calcifications, which are visible on imaging. Radiologists experienced with breast imaging can usually distinguish these from worrying changes. Tell your radiologist that you have had fat transfer.

Am I too slim for fat transfer breast augmentation?

Very slim women may not have enough spare fat to make a noticeable difference. Even small amounts of fat can help with asymmetry or to soften implant edges, but for a meaningful increase in size, enough donor fat is needed.

Can fat transfer replace my implants?

When implants are removed, fat transfer can restore some of the lost volume in the same operation or later. It will not usually replace the full volume of a large implant, but many women are satisfied with a smaller, natural breast.

Is fat transfer to the breast permanent?

The fat that survives is permanent and behaves like normal fat, changing with your weight. If you lose significant weight, the breasts may become smaller, and weight gain may make them larger.
