A thigh lift, or thighplasty, is an operation that removes loose skin and excess fat from the thighs, most often from the inner thighs. It is usually performed after major weight loss, when the skin has been stretched beyond the point where it can recover, but it is also chosen by people whose inner thigh skin has loosened with age. The result is a firmer, smoother thigh with less rubbing and chafing when walking. Like all skin-removing operations, it leaves a scar, which is placed where it is easiest to hide.

Loose inner thigh skin causes both cosmetic and practical problems. It can make fitted trousers and swimwear uncomfortable, cause friction rashes and sweat irritation, and make exercise unpleasant. It does not improve with exercise, because the problem is excess skin rather than weak muscle.

Types of thigh lift

Type Scar Suitable for
Groin crease (horizontal) lift In the groin crease Mild laxity in the upper inner thigh
Vertical inner thigh lift Groin crease plus vertical scar down the inner thigh Moderate to severe laxity along the thigh
Extended vertical lift Vertical scar reaching towards the knee Severe laxity after major weight loss
Outer thigh lift Along the hip, often as part of a body lift Loose skin on the outer thighs and hips

The inner thigh lift is by far the most common. Outer thigh laxity is usually treated as part of a circumferential body lift, which also addresses the buttocks and hips.

Loose skin or fat?

If the thighs are full but the skin is firm, liposuction may be enough. If the skin hangs or folds, especially when you sit or bend your knees, skin removal is needed. Many patients benefit from both: liposuction to reduce the fat and a lift to remove the skin.

Thigh lift after ageing rather than weight loss

Not every thigh lift follows major weight loss. With age, the skin of the inner thigh thins and loosens, even in people whose weight has always been stable. The excess is usually smaller and concentrated in the upper inner thigh near the groin. In these cases, a groin crease lift with a short, hidden scar, often combined with liposuction, may be enough, and the recovery is easier than after an extended vertical lift. The key is an honest assessment of how far down the thigh the loose skin extends, because a short-scar lift cannot treat laxity near the knee.

Who is a good candidate?

You may be a good candidate if you have loose skin on the thighs that causes discomfort or self-consciousness, have maintained a stable weight for at least six months, are in good health and well nourished, do not smoke and accept the scars. After bariatric surgery, a stable weight for a longer period and normal nutrition, confirmed by blood tests, are important.

Preparing for surgery

Stop smoking at least four weeks before and after surgery. Stop blood-thinning medicines and supplements two weeks beforehand after checking with your doctor. Hormonal contraception may need to be paused to reduce the risk of blood clots. After weight loss, your protein, iron and vitamin levels are checked and corrected. Prepare loose clothing and arrange help for the first week.

How the operation is performed

The operation takes two to four hours under general anaesthesia. Markings are made with you standing. Liposuction of the thighs is often performed first. The excess skin is then removed. In the groin crease technique, the skin is lifted upwards and anchored to the strong tissue in the groin, which helps prevent the scar from dropping and the genital area from being pulled. In the vertical technique, skin is also removed from the inner thigh. The incisions are closed in layers, and drains may be placed.

Recovery

  1. Days 1–2: hospital stay; tightness and swelling; walking starts early with short steps
  2. Days 3–10: gradual improvement; compression garment worn continuously; avoid spreading the legs widely
  3. Weeks 2–3: return to desk work for many patients; sitting becomes more comfortable
  4. Weeks 4–6: light exercise
  5. Weeks 6–8: return to running, squats and wide leg movements
  6. Months 6–12: scars mature and swelling fully settles

Hygiene around the groin scar is important in the first weeks. Showering is usually allowed after a couple of days, and the area should be kept clean and dry.

Risks

Thigh lift carries a higher risk of wound healing problems than some other body contouring operations, because the area is under tension when walking and sitting, and because of the moisture and friction in the groin. Possible risks include wound separation, infection, seroma, widened scars, downward migration of the scar, pulling on the genital area, asymmetry, numbness and swelling of the lower legs from disturbed lymphatic drainage. Careful anchoring of the lift, conservative skin removal and good aftercare reduce these risks.

Combining the lift with liposuction

Most thigh lifts include some liposuction, but the two are balanced carefully. Liposuction reduces the thickness of the tissue, which makes the skin easier to lift and the result slimmer. However, aggressive liposuction in the same area as the skin removal can reduce the blood supply to the skin edges and increase the risk of wound problems. The usual approach is to reduce the fat moderately, especially on the inner thigh and around the knee, and then remove the loose skin, rather than trying to achieve the whole result with either technique alone.

Scar care

The scars are red and firm in the first months and gradually become paler and flatter over twelve to eighteen months. Silicone gel or sheets can help once the wounds have healed. Protect the scars from sun exposure. If a scar widens or thickens, treatments are available to improve it.

Life after a thigh lift

Many patients notice the biggest difference in everyday activities: walking without chafing, wearing shorts or fitted trousers comfortably and exercising without irritation. The thighs look slimmer and smoother. Maintaining a stable weight keeps the result.

Thigh lift as part of weight loss surgery

After major weight loss, loose skin often affects several areas. A thigh lift is frequently part of a staged plan that may include a tummy tuck or fleur-de-lis abdominoplasty, an arm lift and breast surgery. Because each is a significant operation, they are usually spaced several months apart, starting with the area that bothers you most or causes the most discomfort.

Thigh lift in Istanbul

International patients should plan ten to twelve days in Istanbul. This allows for surgery, the hospital stay, drain removal and wound checks before the flight. Long flights carry a higher risk of blood clots after surgery on the legs, so wear compression stockings, walk regularly and drink plenty of water.

Thigh Lift: frequently asked questions

Where are the scars after a thigh lift?

For a mild lift, the scar lies in the groin crease. For more loose skin, a vertical scar runs down the inner thigh, sometimes towards the knee. The scars are placed on the inner side of the thigh so they are hidden when standing with legs together.

Does a thigh lift help with chafing?

Often, yes. Loose skin and fat on the inner thighs are a common cause of rubbing, rashes and discomfort when walking or exercising. Removing the excess usually reduces this significantly.

Can liposuction replace a thigh lift?

Only if the skin is firm. Liposuction removes fat but not loose skin. When the skin is loose, removing fat alone can make the thighs look more deflated. A thigh lift removes the skin.

Is recovery painful?

The first week is uncomfortable, mainly because of tightness and pressure when moving and sitting. Pain is controlled with medication. Walking starts early, and most people feel considerably better after two weeks.

How long before I can exercise?

Walking starts immediately. Light exercise usually resumes after about four weeks, and running, lunges, squats and wide leg movements after about six to eight weeks, to protect the scars.

How should I sit after a thigh lift?

Sitting is allowed from the first days, but it can be uncomfortable because of tightness in the groin. Sitting upright on a firm chair with your legs together, rather than slouching or crossing your legs, puts the least strain on the scars. Avoid long periods of sitting without walking breaks.

Will the thighs stay tight?

The removed skin does not come back, and results are long-lasting if your weight stays stable. Some relaxation of the skin over the first months is expected, and the skin continues to age slowly.
